To start off, the horse should have a good halter and lead rope on. The personal assisting should have a good hold and consistently be watching the horse's behavior for the safety of the veterinarian and anyone else there. Another way to keep the horse standing still would be to hold up a front leg. There are hand twitches and tool twitched. The tool can be metal or wood and both are placed over the upper lip and either firmly held closed or twisted tight. The hands twitches can be done by grabbing a handful of neck skin and pinching it tightly. The one I like the least is an ear twitch. Large horses make it difficult to hold onto the ear when you are short and small.
