B! Also, you should explain that you think he may be suffering from a life threatening condition so that they don't think that you are just trying to get them to bring the pet in. The likely diagnosis is GDV, or at the minimum a bloat. I would start setting up IVC materials x 2, IVFs, and make sure radiology is prepped for a large dog. I'd make sure we had a surgical suit available and everything we would need for an abdominal sx on stand-by. Alert the DVM that a possible GDV is coming in so that they are aware a possible surgery in on the way in.
