1.  Place the following in the correct SOAP format:

     Cc: Emergency: Animal comatose

     Wgt = 20#, HR = 200bpm, RR = 120bpm, Temp = 107.5, CRT = 4sec

     Tx: Bathe in cold water

          Alcohol applied to paws and neck

          IV fluids 500ml bolus then 10ml/hr

          Reduce temperature to 102

          Keflin 100mg IV

     Hx: found outside laid out, seemed very hot to touch and panting

     Dx: heat stroke

     PE: skin = hot to touch

          Circulatory = increased heart rate

          Respiratory = increased respiration

     Plan: reduce temperature and administer IV fluids

          Monitor for increase in temperature over next 12 hours

          Monitor condition closely until animal is on its feet, then monitor normally

          Run blood chemistries

 

2.  Place the following into the correct SOAP format:

     Fecal = hookworms and whipworms

     RX: Panacur 50# dose daily mixed with food for 3 days.

     Cc: black tarry stool with mucous

     Hx: loose stool for 5 days

          Black tarry stool for last 5 days

          Stool covered with mucous

     PE: Gastro = fecal residue on rectum, no pain upon palpation

          All other systems WNL

     Tx: Panacur for 3 days

          Repeat deworming in 3 weeks and then 3 months

     Dx: Hookworms and Whipworms

     Temp = 101.2, Wgt = 50#, HR = 80bpm, RR = 26bpm, CRT < 2 sec
