I have some questions to ask that I truly need help with. Before I begin, I appreciate Penn Foster letting us students know how many codes are needed for every scenario. Thank you every much!! Okay... First, there was a question that I came across that was a multiple choice question but you guys said it needed codes! (I trumped that up to a mistake on you guys part).. Okay on Question 4.22, it asks for 3 diagnosis codes for a 5 month old with vomiting, diarrhea & growth failure. The discharge diagnosis was eosinophilic allergic gastroenteritis & colitis. When I get to coding allergic gastroenteritis & colitis, it also asks for another code for what the child is allergic to. First, the scenario doesn't tell you that info & it's confusing if the diagnosis is 2,3 or 4 different words because of how it looks in the tabular than in the index!! I don't even remember learning about allergic gastroenteritis & colitis as one ailment. Can I please get some help on how to code question 4.22?
Next is question 4.32 which asks for 7 diagnosis codes & 2 e-codes.. I'm stuck at 6 diagnosis codes & 1 e-code (accidental puncture). I have the leiomyia, the 3 endometriosis,the blood loss & the adhesions (pelvic). What else I'm I missing? Repairing the puncture as an e-code? Hysterectomy isn't a diagnosis in the index. I'm stuck.
Last is 4.35, which ask for 4 diagnosis codes but I'm doing something wrong because no matter how I do it, I keep coming up with 5 diagnosis codes. A patient with type 1 diabetes mellitus is admitted with acute renal failure. Past history includes hypertension, diabetic nephropathy & chronic kidney disease, stage 4. 1 code is for acute renal failure, With the patient having diabetes & a history of diabetic nephropathy, (a manifestation) that's 2 codes and with the cause & effect with hypertension & CKD (plus having to code the stage of the CKD as 4) is 2 more codes equaling 5 codes. I understand that no ailments say "with" or "due to" but I also understand from what I was taught hypertension & kidney disease, you assume a relationship between the two accept if it's an acute renal failure. Diabetic nephropathy tells you the nephropathy is due to having diabetes. Can you please tell me what I'm doing wrong? I'm pulling my hair out here reading these questions & the guidelines day in & out and I refuse to just send it in and accept points off BUT I am getting discouraged... Please someone help me out....
